Helping Your Patient Become a Donor
Your patient may pre-register to become a donor with LifeLegacy Foundation when pre-planning final
arrangements and death is not imminent. Pre-registration identifies the patient's interest to be
a donor and provides LifeLegacy Foundation with initial information that will assist
with the process at the time of death. Donor acceptance is not determined until the time of death when all the required criteria can be gathered and evaluated.
For pre-planning patient final arrangements, when death is not imminent:
- Contact LifeLegacy at 520-575-5200 or 888-774-4438, or complete our Information Request Form
- Upon receipt of this request, LifeLegacy Foundation will send the patient information regarding donation and the pre-registration form to complete and return.
- Upon receipt of the completed pre-registration form, LifeLegacy Foundation will send your patient confirmation of pre-registration, a donor wallet card, and a packet with additional donation information.
- Your patient should carry the LifeLegacy Foundation donor wallet card to identify him or her as a donor at the time of death.
- Your patient should tell the family about the decision to be a donor.
- Your patient should tell your hospital, hospice or caregiver about the decision to be a donor.
At the time of death or preparing for imminent death:
- Contact LifeLegacy Foundation to complete the necessary paperwork including the donor consent, medical and social history questionnaire, and death certificate information.
- Once acceptance is determined, LifeLegacy Foundation will make all the arrangements to receive the donor and complete the donation.
